Do Not Ride Form

The Do Not Ride Form is intended to notify the  school and Student Transportation Services of
Waterloo Region that you intend to make alternate arrangements for your child to travel to and
from school every day instead of taking the bus.

Joint Custody Transportation Request

This form can only be used if both parents reside within the bus eligible transportation boundary for the school and must be filled out annually.
